I did a qconsole.log download right before it crashed and here is what it said below:

This is Windows 2003 Standard server software running Omaha 24/7 TDM and besides the AP 1.22, the AP Setting's folder, and the Foresight Anti Cheat software;
I have:
1. a basic weapon mod
2. a mustard gas mod - zSpearhead-MustardGas.pk3,
3. a throwing knife mod - zzzz-Throw-Ruckus.pk3,
4. a bayonet mod - zzzzzzuser-Rookie_One_Knife-Combat_Spearhead.pk3,
5. a map mod - user-mefy-extgametypes-1_1_4-SH.pk3,
6. and a server sound fix mod - zzzzzzzzz_serversoundfix.pk3.
The Global localization.txt server crash fix file I removed, cuz I thought the AP had this already incorporated in its file(s), but had left the gamex86.dll server crash Fix (Blackbart's fix #1) on the server.
I'm wondering if the server sound fix (item #6) is calling out for this sound file?
Any help is appreciated.

Try removing that server fix. Oh, and my Knife Combat is a both server- and client-side mod, if the players don't have it, they will see normal pistols, it's just that they will only be able to bash with them.
That was it! The server sound fix was the cause and the source. Found this (sound/dialogue/Generic/A/death/dpl_death_08i.wav1) in the sound file also.
It worked fine...
